Attention: Parents of Students Entering 7th Grade

Florida’s school immunization law requires all students in public schools in kindergarten - 12th grade and public pre-school, including foreign exchange students, to have documentation of proper immunization or exemption to attend school.  Mandatory exclusion from school is required by law in Florida until acceptable immunization documentation is presented.  All immunizations must be presented on a Florida DOH Form 680 – Certificate of Immunization.

 

 

We are encouraging you to make an appointment with your child’s doctor to complete this requirement as soon as possible to facilitate transition to 7th grade.

 

If your child does not currently have a doctor or if your child does not have health insurance, for your convenience in preparing for your child’s enrollment, the Pinellas County Health Department will provide free immunizations. If your child’s immunizations are up to date, please send the record to the school at this time so that your child will be ready for 7th grade enrollment.
